* On July 27, 2011, there was a small fire at Fulks’ home that caused extensive smoke damage to the residence. Polygon was hired by Allstate to remediate the lingering smoke damage. The chemicals used to clean Fulks’ residence were not designed or intended for use in residential premises, the suit says. Fulks is seeking compensatory and punitive damages.

* Jo Ann Adkins was a patient of Haberman’s. On Aug. 30, 2011, blood was drawn from her at HIMG and her blood results were abnormal. On Sept. 7, 2011, Joseph became involved in the medical treatment of Jo Ann Adkins and was aware that she was taking 150 mg of Flecainide twice daily. In June 2012, Jo Ann Adkins suffered ventricular tachycardia and cardiac arrest due to Flecainide toxicity and died on June 14, 2012, the suit says. Willie Adkins is seeking compensatory damages with pre- and post-judgment interest.
